| Annotation | The Collaboration and Assessment with Relatives of Elderly Residents (CARER) model provides a framework for supporting family carers through sharing knowledge and expertise in partnership with others. The CARER model comprises three stages, the first being assessment: obtaining life history information and relevant family history. Second, intervention (telephone contact and review meeting) involves circular questioning and agreeing goals and tasks. Third, evaluation (telephone updates and pre-discharge meeting) provides an opportunity for reflection, validation and future planning. The author uses case study observations and the evidence base to demonstrate the value of the CARER model. (RH). |
