Annotation	This is a summary of a longitudinal study, which compared the process and outcomes of hospital and post-discharge care of older people with stroke or fractured neck of femur in six locations in England. The emphasis was on the views of patients and their carers, focusing on: different approaches to the broad management of care; discharge procedures and factors influencing practice; post-discharge care and the use of health and personal social services; and outcomes six months after admission.
