| Annotation | This commentary questions whether the Step-Down Unit set up in Plymouth (described in the previous article of this issue of `Managing Community Care) reflects service need rather than users' needs, and does little more than achieving savings in hospital acute bed costs. The author suggests that the case study by Savage and Melville raises the following wider issues: can the NHS become user-led; what model of hospital discharge avoids premature decision-taking and keeps costs down; will the Government fund research to provide clear evidence about the effectiveness of such schemes; and will there be recognition that more responsive services are more expensive?
