| Annotation | The main focus of Labour's "New NHS" reform is primary care. The government's intention is both to improve the efficiency and equity of primary care provision and to develop Primary Care Groups (PCGs) and Primary Care Trusts (PCTs), which both provide care efficiently and act as agents who purchase secondary and tertiary care on behalf of patients. This paper explores the policy context of PCGs - that is, how the supply of primary care has developed; and general practitioners (GPs) as purchasers of care. It describes and appraises the government's proposals as presented in the White Paper "The new NHS: modern, dependable" and subsequent guidelines from the Department of Health (DoH) and the NHS Executive. Major issues involved in the implementation of change are identified: organisation size and structure; quality and clinical governance; financial accountability; and rationing. Primary care reforms in the next decade are likely to require increased expenditure, but will also need clearly articulated objectives, shrewd management, and careful performance assessment. (RH). |
